Insurance information

Our insurance coordinators deal with many different insurance companies. Some companies offer different dental and medical plans and some companies combine dental and medical coverage. This insurance alphabet soup changes policies and guidelines weekly. At times, it is almost impossible to accurately estimate our patients' insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance be current.

We have made it easier for you to file your insurance. The receipt form we give you at the end of each visit contains all the information needed from the doctor for your insurance company to promptly process your claim. We file insurance claims for extended procedures as a courtesy to our patients. However, please realize that pre-determination of benefits does not guarantee payment. We must emphasize that YOU are responsible for payment of services rendered, not your insurance carrier. You will be responsible for any amount not covered by your insurance carrier.

Co-Payment

All co-payments are payable when you check in at the front desk. This allows you to directly leave the office following your appointment.
